That is steady work for fabricators, but it also means a lot of first-time buyers are picking a countertop material for the first time, usually while also picking cabinets, paint, and a dozen other things at once.

No — removal of the existing countertop is normally handled as part of a replacement job, not something you need to do beforehand. It is still worth confirming that scope explicitly when you describe the job, since what is underneath the old top sometimes changes what the new install needs.
Sometimes, and it depends on the mounting type and the new material's thickness. A drop-in sink is usually the simplest to reuse if the new cutout matches. Moving to an undermount sink is a different story — it needs a substrate and support built into the cabinet that a drop-in installation never required, so that gets assessed before fabrication rather than assumed.
